Cremation Authorization Form

This Authorization Form must be completed and signed prior to the cremation. Please read it carefully and ask any questions you may have. Cremation is an irreversible and fnal process. We want you to fully understand the information provided so we will be pleased to answer any questions about the cremation process or the other information contained in this Form.  You are responsible for notifying all family members such as  all adult children, if there is no spouse.  Authorization is to be signed by the Next Of Kin, which is the closes living blood relative to the deceased, if signing as the power of attorney, paperwork proving POA will be required.
